I have a facsimile of one in the Japanese mini lp sleeve SHM CD. (Which sounds insane, by the way. "The Moon Upstairs" never sounded like this before.)
The mask is the same shape as the one on the American sleeve. Too big for a Barbie Doll but too small to wear, obviously.
I do have an original UK pressing -- sans the mask and the bombers inner sleeve.
But I do have a music press ad with the bombers design, weirdly superimposed by the cover design of the original "The Velvet Underground" paperback cover.
