Having combined cycle pedestrian paths just like Amsterdam and Hamburg being examples is the way to go. It becomes ingrained that having equal priority makes both parties watch out for each other - the other extreme which kind of works is the free for all of road users in India as everyone drives and rides defensively. There have been experiments (I will try and find a reference) I think in Denmark where Traffic lights and similar relevant road furniture have been removed and just like India everybody looks out for each other, but without the mayhem which eventually does occur in India. As for Bradley Wiggins and the man with 3 legs; most professional bike riders don't ride a bike unless they are racing or in controlled environment. Even Carl Fogarty in his biography said about riding a motorbike that he does not unless he is at work. He drives a car instead and advises to watch your speed.
